Stroke Rehabilitation at Home: Mobility and Independence

Stroke rehabilitation at home is a personalised process that helps a person practise meaningful movement, transfers, balance, walking, and daily tasks as safely as possible.

Stroke rehabilitation prioritises meaningful, repeatable tasks rather than isolated exercises alone. The therapist can work with the person and caregiver on bed mobility, transfers, walking, upper-limb use, fatigue, and falls prevention.

Expected prognosis

Stroke recovery varies widely with the location and severity of the stroke, medical complications, cognition, fatigue, support, and time since onset. Progress is best tracked through meaningful function rather than a single symptom or test.

Approximate time

Rehabilitation begins when medically appropriate and may continue for months or longer. Review frequency changes as safety, independence, and the person’s goals evolve.

Safety notes

  • Sudden new weakness, facial droop, speech or vision change, severe headache, loss of balance, or confusion requires emergency stroke assessment.
  • Exercise must be adapted for falls risk, fatigue, blood pressure, swallowing, and other medical needs.
  • Caregivers should receive transfer instruction rather than lifting or pulling without training.
